OHF Requirements for 2021 – 2022 Season

Hello Parents,

We have been advised by the Ontario Government, Ontario Hockey Federation (O.H.F.) and the Greater Toronto Hockey League (G.T.H.L.)  that prior to any players being eligible to play in the upcoming 2021/2022 season,

Please review documents, sign and return the OHF ROWANS LAW Acknowledgement Form to registrar@eastyorkhockey.org as soon as possible.

If you do not have access to a scanner, you can bring the signed copy with you to the Arena.  You can hand in completed form at the Convenors Bunker downstairs at the Arena (room located next to the skatesharpening )

Note that there are different documents for each age level so be sure to select and share the appropriate document based on your child’s age.

Once we receive signed copy, we will add the authorization form to the your child’s HCR file (Hockey Canada Registry) to complete the obligation to both the O.H.F. (Ontario Hockey Federation) and the G.T.H.L. (Greater Toronto Hockey League).

All players are required to have a form on file with the League Office to be eligible to participate in any games during the 2021-2022 Season
